Privacy, cancellation and complaints
This page states what this website does with the information you type into it, and what the clinic's cancellation and complaints process is. It is a draft for the clinic to review.
Privacy
What this website collects
Only what you type into one of the two forms: the booking request form and the enquiry form. That is a name, a phone number, an optional email address, an optional note, and for a booking the service, date and preferred time you chose.
What it does not collect
No account, no password, no payment details, no medical records. There is no patient portal and no login on this site.
Who receives it
Form submissions are delivered to the clinic's reception by email through a form-handling service. [Client to confirm: the exact handler, e.g. Formspree or a Cloudflare Worker, and who at the clinic receives the messages.]
How long it is kept
[Client to confirm: a retention period, for example "form emails are deleted after three months". Do not publish this page without one.]
Do not send medical details through either form. They are not a secure channel for health information. Call the clinic instead.
Data handling
- No analytics on this site. Nothing records which pages you visited or how long you stayed.
- No session recording. No heat maps, no replay tools, no scroll tracking.
- No advertising cookies. The site sets no cookies of its own.
- Fonts are loaded from Google Fonts, which means your browser contacts Google to fetch two typefaces. If the clinic prefers, the fonts can be self-hosted so no third party is contacted. [Client to decide.]
- No patient data is used for any other purpose. Nothing typed into a form is added to a marketing list.
Cancellation and no-shows
Call the clinic as early as you can if you cannot attend. That time can then go to another patient.
[Client to supply: notice period, whether a fee applies to a late cancellation or a missed appointment, and the exact wording they want published. The paragraph above is a placeholder and must not go live as the whole policy.]
A booking made through this website is a request until reception confirms it by phone. A request that is never confirmed is not an appointment, and no fee can apply to it.
Complaints
If something went wrong, tell the clinic. Ask for the practice manager and explain what happened. You can do this by phone, in person, or by email.
Named contact for complaints: [Client to supply: name and role of the person who handles complaints]
Telephone: 0800 000 0000
Email: [Client to supply: complaints email address]
[Client to supply: how long a complaint takes to be answered, and what happens if the answer is not accepted.]
